So, if you are on a live-aboard, you need 50 logged dives, insurance and
a medical cert.
But, if you are on a shore based holiday, with a 3 or 5 day package, you
don't need any of those, and the onus is on the operator of the boat to
ensure there are enough guides based on the experience level of the
passengers.

How is an Egyptian operator going to know you didn't sign the medical cert
yourself?

Your insurance is the important thing. As long as you satisfy the medical
requirements for that, you should be covered.
