This is clearly new and somewhat confusing.

I have just booked some diving with Emperor Divers in Sharm, and
specifically mentioned the changes and asked for the form/format.

The reply said that the rule didnt apply there yet, but if we were
concerned, a letter from a GP confirming that I am fit to dive would
suffice.

They may be wrong, or may not be aware of the changes, or something -
but if no-one can find the prescribed forms, or identify the specific
requirements, then something is wrong, (it is Egypt, so the latter is
possible <g>).

Anyone just come back from Egypt and know the current situation regarding
the above?

I am off to dive around Hurghada in April and am wondering what the latest
news is from the coal/coral face for medicals required!

Surely someone has some information!
