They certainly don't win all their cases and they certainly don't all get paid millions. And there's nothing preventing the client from suing on his own if he doesn't feel the attorney is worth his fee. After all, there's no monopoly on suing, unlike the monopoly on medical care practiced by the evil doctors and the AMA.
