Default Safe diver, dangerous or just plain stupid

I have just spent the last evening in the pub catching up on a recent
trip to Scapa Flow that I wasn't able to go on. During the conversation it
came to light that one of the divers who I have dived with in the past did
something that I couldn't believe. The diver in question is a padi
divemaster and a IANDT Normoxic Trimix Diver. On the first day I have been
told the individual bent a suunto vytec computer in regards to not switching
gases on it on the first dive. Then on the second dive on that day bent
there VR3 by passing a 15m ceiling due to watching a wrasses that was
passing by. With both computers locked out the following days dive were done
either on tables or just sheer good luck.

So I was just wondering if this is a common problem and which one of the
three categories in the subject would this diver come under
